SchoolDoc is a user-friendly, web-based electronic health record (EHR) that is used on both the Lower School/Middle School and Upper School campuses to store and document any student medical issues, allergies, illness, injury, and treatment provided by the MBCA school staff. Full completion of SchoolDoc prior to the start of each school year is mandatory for school attendance. For more information on SchoolDoc, click here
Prior to the start of each school year, parents will receive an email invitation to complete your child’s Health History in SchoolDoc. Each year, you must initially enter SchoolDoc via the invitation sent to you - please do not try to enter SchoolDoc via any previously saved links or attempt to create a profile on your own. Failure to enter SchoolDoc via the invitation each year will result in your child’s information (even if you are able to input information) from being uploaded to the program for the current school year.
It is imperative that your child’s Health History in SchoolDoc be kept up to date by you at all times. This is the only way MBCA can remain aware of your child’s medical issues, allergies, medications they are allowed to receive while at school, and who to contact in the event of a serious illness/injury or emergency situation. If any updates to your child’s health, allergies or medications occurs, please make sure to update this in SchoolDoc.
Only the MBCA Clinic Nursing Staff (and very select authorized staff at the Upper School) will have access to your child’s medical records; your privacy is very important to us and will be handled in a professional manner. SchoolDoc does comply with FERPA and HIPAA regulations. Please answer all questions truthfully and to the best of your ability, knowing that your information will be safe and protected.

ACTION PLANS
If your child has been diagnosed with severe allergies, asthma, seizures or diabetes that would require Clinic or Staff intervention, you will need to provide an Action Plan signed by your child’s physician prior to the start of the school year (or at any time during the school year that a diagnosis has been made by any medical provider).
Please inform the Clinic Staff if your child has any food, medication, or environmental allergies by indicating so in your child’s SchoolDoc Health History. If your child has any allergies that require an Epi-pen, Auvi-Q or antihistamines, you must provide MBCA with a completed Allergy Action Plan that is signed by your child’s medical provider. You will need to provide the MBCA Clinic with two epinephrine auto-injectors and/or any antihistamines your child would require in the event of an allergic reaction.
It is also highly recommended that you make an appointment to meet with the Director of the MBCA Cafeteria to discuss your child’s allergies. Please contact the MBCA Clinic for additional information.
Please inform the Clinic if your child has asthma by indicating so in your child’s SchoolDoc Health History. You must provide MBCA with a completed Asthma Action Plan that is signed by your child’s medical provider. You will need to provide a rescue inhaler (Albuterol/Levalbuterol) and/or nebulizer medications for your child to be used in the event of an asthma attack.
Please inform the Clinic if your child has been diagnosed with any seizure disorder (absent, focal, petit mal, grand mal, or PNES - Psychogenic Non-Epileptic Seizures) by indicating so in your child’s SchoolDoc Health History. You must provide MBCA with a completed Seizure Action Plan (or PNES Action Plan) that is signed by your child’s medical provider. You will also need to provide any rescue medications your child may require in the event of a seizure that occurs while at school.
Please inform the Clinic if your child has been diagnosed with any form of diabetes (Type I/Type II, insulin or non-insulin dependent) by indicating so in your child’s SchoolDoc Health History. You must provide MBCA with a completed Diabetic Action Plan that is completed and signed by your child’s medical provider. You will need to provide any diabetic supplies including a glucometer, glucometer strips, ketone strips, insulin, medications and/or food/drink that your child would require in the event of a hyperglycemic or hypoglycemic event that occurs at school. You will be provided with a sample of the school lunch menu and are required to provide the Clinic with carbohydrate calculations based on what your child may normally eat for lunch. You will also need to make an appointment to meet with our Cafeteria Supervisor - Cathy Cuhna. Please contact the MBCA Clinic for additional information.
** The Clinic does have the ability to monitor your child via a continuous glucose monitoring system and insulin pump if your child requires this. Please contact the Clinic for additional information.
IMMUNIZATIONS
Your child may not attend school at Mt. Bethel Christian Academy until an up-to-date GRITS immunization form 3231, medical waiver 3231, or signed and notarized religious waiver 2208 has been provided to the MBCA Clinic.
This is required by law (GA Code 20-3-771, updated 2022)
for school attendance.
All students in the state of Georgia, are required to have a Georgia Registry of Immunization Transaction Services (GRITS) GaDPH Form 3231 to be eligible to attend school. An up-to-date copy of this form must be provided to the MBCA Clinic prior to the start of each school year.
SchoolDoc will prompt you to upload a copy of your child’s immunization record and document the current expiration date. If your child has received all required childhood immunizations (all three boxes are checked in the upper right hand corner of GaDPH form 3231), you may enter May 31 of the appropriate graduation year for your child as an expiration date.
For students requiring a medical exemption for immunizations, please contact your physician to have them complete the medical exemption(s) on your GRITS - GaDPH form 3231 and upload this to SchoolDoc. A note from a physician is not acceptable. Medical exemptions are only valid for one year and must be updated each year by your child’s physician.
For students with a religious exemption for immunizations, a notarized Affidavit of Religious Objection to Immunization (GaDPH form 2208) must be uploaded to SchoolDoc prior to the start of school.
Students who have not attended school in the state of Georgia within the previous 12 months are granted a 30-day grace period to submit the required GaDPH forms. This includes students transferring from other states and countries. THIS IS THE ONLY GRACE PERIOD ALLOWED FOR STUDENT IMMUNIZATION RECORDS.
Parents will need to establish a local physician or make an appointment with the local Health Department to have immunization records transferred into GRITS. The 30-day grace period begins on the first day of school.
Your child WILL NOT be allowed to attend school on the first day if these documents have not been received by the MBCA Clinic. If you have extenuating circumstances, please contact the MBCA Clinic for further guidance.
If your child’s immunization “grace period” expires, and you have not obtained a GA immunization record, your child will be sent home and will not be able to return to school until this has been completed, AND a copy of the immunization record has been turned in to the MBCA Clinic.

MEDICATION ADMINISTRATION FORMS
Parents must complete a separate Medication Administration Form for each medication submitted to MBCA to be dispensed to a student while at school. This form template can be found in SchoolDoc, as well as on the MBCA website, and must be uploaded to SchoolDoc by the parent.
